Registering as a Patient
If you live in our practice catchment area you can join our list.
We are using a new online service that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

All are welcome.
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services.

If you wish to register, please speak to a member of staff who can help and support you with the process of registration. Ask reception for an interpreter if you find it difficult to communicate in English.
All organisations providing NHS and adult social care are legally required to fully implement and conform to the NHS England Accessible Information Standard.
This means that Practices must ensure that patients who have a disability, impairment or sensory loss are able to receive information that they can access and understand.
Please get in touch if:
- You need written information in an alternative format, eg large print or easy read
- You need help to communicate with us, eg because you use British Sign Language
We also offer help such as:
- Appointments with a member of our team who has extra training to support your communication needs
- Appointments at times when the waiting room is quieter
- The support of an advocate
- Longer appointments
- Appointments scheduled for when your advocate or carer can attend with you
